Prospective Student Information Forms
If you are interested in placing your child on the waiting list, please complete and submit the appropriate Prospective Student Information Form listed below. Once submitted, be sure to go to your email and verify the email sent to you to complete this process. When we receive the Prospective Student Information Form, we will add your child to the Waiting List until space is confirmed for enrollment, as well as start date. You will receive a notification by email once a space becomes available. IMPORTANT: Please be sure to have come out for a visit within 30 days of submitting your Prospective Student Information Form.
